Ministerial Credentialing is an ever important issue. We are here to serve you and as a result have made the following frequently requested information available for download. Please review this information carefully before requesting a Credential Application. After requesting your application we will then email you the proper form. We will evaluate your application and respond to you very soon.

| PHASE 1: Application
PHASE 2: Signature Packet
- The Signature Packet with instruction letter are available at Game Days or will be mailed to you. (Return in 5 days.)
PHASE 3: Reference Packet
- The Reference Packet will be mailed to you. (Request References be returned in 10 days.)
PHASE 4: Credential Exam
- First Saturday of the month at District Office
- Or schedule Exam through deana.smith@okag.org
